Person Specification: Children & Young People's Link Worker
ROLES & RESPONSIBILITIES
Key duties
1. To provide personalised support
- person-centred and creative one-to-one support for CYP exploring what matters to them
- link CYP and families to services and community support that will best meet needs
- work holistically with CYP and families including addressing any barriers to engagement in services or activities
- work collaboratively with CYP and families helping to empower participants to take positive action for their own wellbeing
- facilitate ‘soft’ introductions to groups and activities where appropriate
- manage and prioritise a caseload of participants, ensuring that ambitious performance targets and project objectives are met
2. Support community groups and VCSE organisations
- develop supportive relationships with local VCSE organisations, community groups, and statutory services
- constantly develop and hone the asset map of networks, services, and community groups that support CYP and families
- work with the CYP’s Services Manager to identify gaps in provision and to design creative ways to fill them
- encourage CYP to volunteer, with the aim of building their own skills and confidence, and to strengthen community resilience
3. Data capture
- keep accurate records of service delivery including contributing to the collection of monitoring information and preparation of progress reports
- follow agreed processes and protocols for receiving, storing and transferring information about participants, and to ensure that confidentiality is maintained – completing all necessary administration in a timely and comprehensive manner
4. Professional development
- work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the roles and responsibilities
- Access regular clinical supervision to enable you to deal effectively with the difficult issues that people present
General duties
- work as part of the team to seek feedback and continually improve the service
- work in close partnership with other Trust projects to ensure joined-up working and smooth referral routes
- maintain a professional attitude and conduct
- have a flexible approach to working which will include occasional evenings and weekends
- undertake any other reasonable additional tasks

ESSENTIAL
Qualifications
- Education to Level 3
Experience
- at least 2 years in a role aimed at supporting holistic wellbeing and health
- of working holistically, on a one-to-one basis, with CYP with poor mental health and wellbeing
- of recording and monitoring service data
- of working from an asset–based approach: building on existing community and personal assets
- of working with the VCSE sector, including with volunteers and small community groups
- of partnership/collaborative working and of building relationships across a variety of organisations
Skills/Knowledge
- a genuine passion, empathy, and desire to support CYP to lead healthier and happier lives
- ability to listen to, and empathise with, CYP and provide person-centred support in a non-judgmental way, respecting lifestyles and diversity, and inspiring trust and confidence.
- excellent written and verbal communication skills
- confidence in having difficult conversations
- ability to develop and maintain partnerships with a range of professionals and stakeholders, promoting collaborative working and finding creative solutions to community issues
- excellent IT skills, including word processing, spreadsheets, email, and internet research
- ability to work independently and proactively on own initiative, and to work flexibly and enthusiastically as part of a team
- excellent organisational skills, with the ability to complete administrative tasks effectively and efficiently, and to organise, plan, and prioritise on own initiative, including when under pressure and meeting deadlines
- understanding of health inequalities/social determinants of health, and knowledge of community-based interventions that support residents to lead healthier and happier lives
- an understanding of child development
- knowledge of community development principles
Other
- commitment to reducing health inequalities and proactively working to reach CYP from all communities
- able to demonstrate personal accountability, emotional resilience, and to work well under pressure
- willingness to undertake any other duties associated with the role
- willingness and ability to work outside normal office hours and across multiple sites

DESIRABLE
Qualifications
- relevant health, wellbeing or youth related qualification
- trained in Motivational Interviewing
Experience
- proven track record of working successfully with health providers, schools, youth organisations, and community groups, to deliver outcomes
- Youth Worker and/or Social Prescribing Link Worker experience
Skills and Knowledge
- an understanding of health, social care and voluntary sector provision, as well as the challenges currently faced, and the issues affecting, local communities
Other
- knowledge of Southmead and North Bristol, and the issues facing local CYP
- knowledge of VCSE, community and statutory services in the area
