Reporting to the Social Prescribing Link Worker Manager you will be joining our social prescribing team, working in GP practices across North Bristol. You will work with people to identify their needs, then support them to access other organisations, services or community groups as appropriate.

Salary: £21,094-£25,210 per annum pro rata (subject to current job evaluation process scheduled for completion summer 2021)

Hours: up to 37 hours
Contract type: Fixed term until 31st March 2024
